YOUR APPOINTMENT WITH DR SAM MARTIN
To book your initial appointment with Dr Martin, you’ll need a current referral from your General Practitioner (GP) or another specialist, if applicable. This referral allows you to claim a Medicare rebate for your consultation. Keep in mind that referrals do expire, so you may need a new one for any follow-up visits.
Once you have your referral, please contact our rooms to arrange your appointment date and time:

Our team will securely collect any relevant medical information to assist in your care. Where appropriate, we may liaise with your referring doctor prior to your visit to ensure we have a complete understanding of your condition.
- A referral from your GP (or specialist)
- Your insurance information: Medicare card, DVA card, or private medical insurance details
- Your medical records: Copies of previous operation records, medical records, x-rays, MRIs, CT scans.
- Any physiotherapist reports: Progress letters from physiotherapists, if applicable.
- A list of any previous surgery records: Operation reports and discharge summaries from previous surgeries.
- To help streamline your first appointment, we encourage you to complete the New Patient Form in advance. This form includes basic personal details, your medical history, current medications, and any known allergies. By submitting it ahead of time, we can better prepare for your consultation and ensure you receive the most appropriate care.
When attending your consultation, please wear comfortable, loose-fitting clothing that allows easy access to your hip or knee for examination e.g:
For knee assessments: Wear or bring shorts, or loose pants that can be rolled up easily above the knee.
For hip assessments: Comfortable pants or athletic wear that can be adjusted or lowered at the waist may be helpful.
Ladies: Shorts, loose leg slacks or skirt without stockings.
Gentleman: Loose leg trousers or shorts.
You may be asked to walk or perform simple movements during your examination, so it’s best to avoid tight or restrictive clothing.

During your visit, Dr Martin will discuss your medical history and perform any necessary examinations, tests, or scans to evaluate your condition. Based on the findings, he will recommend the most appropriate treatment options for you. To ensure clarity and comprehensive care, he may also provide feedback directly to your GP or referring doctor during your appointment, allowing you to ask any questions and understand the next steps in your treatment plan.
Following your consultation, we will communicate with your GP or healthcare provider to determine the best approach for your case. If surgery is required, we will select the appropriate hospital and anaesthetist to ensure optimal care. Should surgery be deemed necessary, you will need to schedule follow-up appointments to discuss the procedure in detail, address any additional questions, and complete consent forms.
If further imaging or treatment is required, our team will help you coordinate follow-up visits. Review consultations may be scheduled to assess your progress, evaluate imaging, or discuss surgical planning and recovery.

If your injury is work-related, you will need prior approval before booking. Please contact our rooms with your claim number, insurer name, and your case manager’s contact details so we can assist you in coordinating your consultation

Dr Martin offers reduced fees for pensioners and healthcare card holders, ensuring high-quality care is accessible to everyone.
Payment options by location
EFTPOS payment is available on-site at our Grafton clinic. For all other consulting locations, payment is required over the phone via EFTPOS after your appointment. Emma will contact you directly to finalise payment.

If you don’t have private health insurance, you may choose to self-fund your surgery. During your consultation, Dr Martin will explain the available options and provide a detailed breakdown of the associated costs.
